Suppose fictional Stonebridge Credit Union has repeated customer complaint spikes after system releases. An adversarial audit team might issue a broad finding that management failed to test properly. A stronger audit team defines criteria, reviews release controls, tests a sample of changes, identifies that emergency changes bypass user acceptance review, and recommends a practical escalation control for high-risk releases.

Independence does not require hostility. Objectivity means the auditor does not subordinate judgment to management's preferences. It does not mean ignoring context or refusing to understand operational constraints. In fact, good recommendations usually require understanding why the process failed.
