If parents give cash to an adult child, when is there actually a gift tax problem?

People always say a certain amount can be gifted tax free, but I feel like that phrase creates more confusion than clarity. I am not sure when a return is required, when the recipient owes nothing, and when lifetime exemption rules enter the picture.
